Do you take insurance?
We are a 100% self-pay clinic and do not accept any insurance. Payment can be made with all major credit/debit cards and HSA/FSA cards. Care Credit is not accepted. Payment is due at the time of service.

Do you offer any discounts?

Yes, we offer a 10% student, teacher, military, and healthcare worker discount with ID on a la carte appointments only, not applicable to packages. Please let us know at the time of booking and the discount will be added to your account for all future billing.

What is the cancellation/no-show policy?

We understand that life happens which is why we offer a 24-hour cancellation/reschedule window with no penalty! Any cancellation/reschedule with less than 24 hours notice or a no-show will incur a fee of $75. There is a 15 minute grace period for late arrivals for all appointments but your appointment time will be shortened.

Can you see or consult with my child?

Dr. DiStefano works with clients ages 18 and up. 

 

I don't live in Arizona, can we still work together?

Yes! If you live outside the state of Arizona, you can still receive consulting services from Dr. D.

Consulting services include nutritional and lifestyle counseling, advice on building a care team and advocacy, and answering your questions about management of hypermobility and related conditions. Consulting services are NOT medical services and will not include any diagnosis, interpretation or ordering of labs/imaging, or prescribing.

Can you be my PCP (Primary Care Provider)?

As Dr. DiStefano’s services are focused on hypermobility and related conditions, she cannot be your PCP. All clients are required to have a regular primary care provider on your care team while working with Dr. D.

What happens if I’m already Dr. D’s patient? 
You are more than welcome to continue care with Dr. D at The Bendy ND. You do NOT need to schedule another New Patient visit, but you will need to come in to the office in Mesa for an updated physical exam (10-15 minutes, at no cost to you) to continue medical care! Scheduling this will be discussed with you at your first telehealth follow-up visit.

What do the New Patient Screening Labs include?

The screening labs include: CBC, CMP, thyroid panel (TSH, Free T3, Free T4), lipid panel, A1c, Fasting Insulin, iron panel (total iron, TIBC, ferritin), AM Cortisol, CRP, Serum B12 and Folate, Magnesium, and Vitamin D. This is a much more extensive panel than typical screening labs!

The screening lab panel and review report is included with the Founder's Package but can be added to any appointment at an additional cost (lab cost ~$200 + $100 Lab/Testing Review Report).

Additional labs and/or specialty labs including mycotoxin testing, GI/Stool panel, genetic testing, etc may be recommended by Dr. DiStefano and can be added at additional cost (lab/testing cost varies + $100 Lab/Testing Review Report).

All labs and specialty testing are self-pay only (credit, debit, HSA/FSA), no insurance accepted.

Will I get my blood drawn at the office?

No, labs ordered by Dr. D are collected through Ulta or Evexia Labs, who have convenient draw stations located all around the Valley. Specialty labs are typically collected using at-home kits mailed directly to you.

I'm interested in the Founder's Package, what's the fine print?

Great choice! The Founder's Package is the most cost-effective option if you are a new patient seeking comprehensive diagnosis and management of hypermobility/EDS and related conditions with total savings of $300!

• Available to New Patients only.

• Arizona clients only.

• Full package price is due upfront.

• All visits included in the package will expire within 14 months of the purchase. 

• No substitutions or changes can be made to the package (for example, you cannot "remove" the genetic testing).

• The package is non-refundable and non-transferrable.
